Reference materials for the course: You will need to download and either print out the document or you may choose to fill in the form using MS Word.
First Video is titled Phone Appointment.
Second Video is titled Tele form. There is a special form that is required and is only obtainable from the Coach directly. Simply click on the link: Email the Coach and the system should open your email program with the subject line already filled in.
